Hepatitis B Surface Antigen (HBsAg) is the earliest indicator of an active Hepatitis B virus (HBV) infection. Its presence in the blood indicates a current Hepatitis B virus infection. HBV DNA PCR and other hepatitis markers are used to assess viral replication, infectivity, and disease stage. A positive HBV result in this screening test needs further confirmation by a HBsAg quantitative test by ELISA or CMIA method. This qualitative test is suggested for a primary screening purpose before a surgery, during pregnancy, as part of a premarital health checkup, and a routine preventive health checkup.
- HBsAg Quantitative Test (CMIA/ELISA): Measures the exact concentration (titre) of the surface antigen in the blood. This highly sensitive, lab-based test is essential for establishing a baseline, monitoring disease progression, evaluating the effectiveness of antiviral therapy, and confirming chronic infections. A positive report does not confirm a diagnosis, and all positive cases should be confirmed by a confirmatory test like PCR. In some cases and in an unknown percentage of subclinical hepatitis B virus infections, the antigen can be detected in the serum for years, if not for life. Despite the high sensitivity of HBsAg fully automated Chemiluminescent Microparticle Immunoassay (CMIA), the risk of transmission of hepatitis B by an HBsAg-negative sample cannot be ruled out.
