The second-trimester screening risk is calculated using maternal serum marker assays: AFP (maternal serum alpha-fetoprotein), β-hCG (beta subunit of human chorionic gonadotropin), and uE3 (unconjugated estriol) for the Triple Marker. For the Quadruple Marker, Inhibin-A is additionally included.

The numerical risk estimate is generated by the software program SsdwLab6 using a statistical database and patient-specific inputs such as maternal age, weeks of pregnancy (WOP), maternal weight, gestational age (as per USG), number of fetuses, obstetric history, medical history, IVF pregnancy information, and relevant demographics.

Because the calculation is statistical, any risk indicated is not confirmatory evidence of fetal abnormality. A screen-positive risk indicates that further clinical evaluation and confirmatory testing will be required. The report should be interpreted along with ultrasound findings and other clinical/laboratory evidence, as advised by the obstetrician.

The calculated risk depends on the accuracy of the USG/NT (U.S. scan) details and the maternal details provided. Incorrect or incomplete information may significantly alter the risk estimate. The laboratory participates in UKAS proficiency testing (EQAS) for maternal serum markers; however, the laboratory cannot be held responsible for the impact of incorrect clinical inputs on risk assessment.

Clinical limitations that can affect risk estimation
  • Triplets or higher-order gestations: risk calculation cannot be performed.
  • Twin pregnancy with fetal demise (vanishing twin): risk estimation may be possible but can be unreliable.
  • Method variation: different screening software and assay methodologies may yield varying risk estimates.
  • Risk at term vs risk at sampling: both are valid approaches, but values can differ because a correction factor for intrauterine mortality is applied for risk at sampling and may not be applied for risk at term.
