Clinical Significance of the CRP Test
C-Reactive Protein (CRP) is a protein present in the sera of acutely ill patients that binds the cell wall C-polysaccharide of Streptococcus pneumoniae and agglutinates the organisms.
As one of the strongest acute-phase reactants, CRP plasma concentrations rise significantly in response to:
- Infection (concentrations are generally much higher in bacterial than in viral infections)
- Inflammation and physical stress
- Tissue trauma or post-surgery recovery
- Myocardial infarction (heart attack)
- Neoplastic proliferation
Interpretation & Clinical Utility
- Active Inflammation: Concentrations > 5 to 10 mg/L strongly suggest the presence of an active infection or inflammatory process. The peak increase is directly proportional to the extent of tissue damage.
- Disease Monitoring: It is highly useful for screening the activity of inflammatory diseases (such as rheumatoid arthritis, SLE, and leukemia), monitoring post-surgery recovery, detecting rejection in renal allograft recipients, and identifying neonatal septicemia and meningitis.
Important Caveat: CRP is a nonspecific marker. An elevated level indicates that inflammation is happening in the body, but it does not identify the exact cause or location. Results cannot be interpreted in isolation and must be evaluated by a certified medical professional alongside other laboratory findings and a detailed clinical history.
