Total T3 and T4 vs Free FT3 and FT4

Total T3 and Total T4 measure the protein-bound and unbound forms of these hormones in the blood. FT3 and FT4 measure only the unbound or free thyroid hormones.

Including both total and free thyroid hormone tests provides the total circulating hormone values and the unbound hormone fractions. A doctor can interpret these results with UTSH, symptoms, medicines, pregnancy status, medical history, and earlier thyroid reports.

What Is Ultrasensitive TSH?

Ultrasensitive Thyroid-Stimulating Hormone (UTSH or us-TSH) is a third-generation TSH test that can reliably measure very low TSH concentrations. Its higher analytical sensitivity helps identify suppressed TSH levels more precisely than older-generation TSH assays, which is important in thyroid function assessment and hyperthyroidism evaluation.

FAQs on Advanced Thyroid Test Results

Can a normal T3 result rule out a thyroid disorder?
No. T3 can remain within the laboratory reference range in some thyroid conditions, particularly hypothyroidism. UTSH, FT4, other thyroid test results, symptoms, and medical history should be interpreted together.
What can a high UTSH result with a normal FT4 result indicate?
This can be seen in subclinical hypothyroidism, where UTSH is raised but FT4 remains within the reference range. A doctor may advise repeat testing and consider symptoms, medicines, pregnancy status, recent illness, and thyroid antibody tests before deciding the next step.
What can a low UTSH result with normal FT3 and FT4 results indicate?
This can be seen in subclinical hyperthyroidism, but medicines, pregnancy, recent illness, and temporary changes can also affect thyroid values. The result requires medical interpretation rather than assessment from UTSH alone.
Why can Total T3 or Total T4 be high when FT3 and FT4 are normal?
Total T3 and Total T4 include hormones attached to blood proteins. Pregnancy and estrogen-containing medicines can increase thyroid-binding proteins and raise total hormone values while the free hormone values remain within the reference range.
Can an illness temporarily change thyroid test results?
Yes. An illness can temporarily alter T3, T4, or UTSH even when the changes are related to the illness rather than a thyroid disorder.
