What Are H. pylori Antibodies?

Helicobacter pylori is a prevalent spiral-shaped bacterium that colonizes the human stomach lining. It is a primary cause of chronic active gastritis and peptic ulcer disease, and is recognized as a major risk factor for gastric cancer.

When you are exposed to this bacterium, your immune system defends the body by producing specific proteins called antibodies. An H. pylori serology (blood) test does not look for the bacteria itself; rather, it measures the concentration of these antibodies (IgG, IgA, and IgM) in your blood to determine if your immune system has mounted a defense against an H. pylori infection.

Clinical Significance of IgG, IgA, and IgM Markers

Depending on your symptoms and clinical history, gastroenterologists may order a single IgG marker or a combination of all three immunoglobulins to evaluate the timeline and nature of your immune response.

Antibody MarkerDiagnostic Role / Clinical Association
H. pylori IgGThis is the most clinically requested marker. IgG antibodies appear a few weeks after the initial infection and remain detectable in the blood for months or even years. A positive IgG result confirms that you have been exposed to H. pylori at some point, but it cannot differentiate between an active infection and a past (cured) infection.
H. pylori IgAIgA is the predominant antibody found in mucosal secretions (like the stomach lining). IgA is associated with mucosal immune response to H. pylori and is interpreted along with symptoms, clinical history, and other findings. It does not confirm active infection by itself.
H. pylori IgMIgM is the first antibody produced during an acute, primary immune response. It is rarely elevated in adults (as most infections occur in childhood) but can be useful in identifying a very recent or early-stage acute primary infection.

Important Clinical Limitation: Because IgG antibodies can remain elevated long after H. pylori has been successfully eradicated via antibiotics, this blood test is not recommended to confirm if a treatment was successful (eradication testing). Clinicians typically prefer urea breath tests or stool antigen tests for post-treatment confirmation.

Laboratory Methodology & Process

Thyrocare conducts this test using immunoassay technology. The standard testing protocol includes:

  • A trained phlebotomist collects 3 mL of venous blood.
  • No fasting is required. You can eat and drink normally before the sample collection.
  • The serum is extracted and processed using the ELISA (Enzyme-Linked Immunosorbent Assay) method, which measures the optical density (OD) ratio of the antibodies.
  • Strict internal quality control (QC) checks are performed.
  • Digitally verified reports are released within 24 to 36 hours of sample collection.

FAQs: H. pylori Blood Tests

What does the H. pylori IgG blood test measure?
It measures the presence and quantity of IgG antibodies against Helicobacter pylori in your bloodstream using the ELISA (Enzyme-Linked Immunosorbent Assay) method. The results are typically reported as an Optical Density (OD) ratio.
Is fasting required for this test?
No preparation or fasting is required for the H. pylori antibody test. However, if it is booked alongside other diagnostic tests (like fasting glucose or a lipid profile), you should adhere to the fasting requirements of those specific tests.
What sample is collected and how is it collected?
A standard venous blood sample is required. A phlebotomist will draw approximately 3 mL of blood from a vein in your arm.
Do you provide home sample collection?
Yes. Free home or office sample collection is provided across 2,500+ PIN code areas in India, arranged as per the convenience of the beneficiary.
When will I receive the report?
The standard reporting turnaround time is 24 to 36 hours from the time the sample is successfully collected.
Can this antibody test confirm an active infection?
This is a serological test that detects an immune response (antibodies), which can persist long after an infection has cleared. Therefore, while a positive result confirms exposure, it cannot reliably differentiate between a past, cured infection and an active one. A clinician must decide if a urea breath test or stool antigen test is required for further assessment.

Note: This information is strictly for educational and test booking purposes. H. pylori antibody tests cannot confirm if an infection is currently active or if eradication treatment was successful. Always consult a gastroenterologist or physician to correlate these results with your clinical symptoms and medical history.
